Terms and conditions for Monthly Membership

MONTHLY MEMBERSHIP AGREEMENT

This contract is made between the client and FIT4MOM Rockland. This membership is a month-to-month agreement. This contract may be terminated by either party with the proper advanced notice once required payments have been made. This membership allows you to attend unlimited Body Ignite and Stroller (Strides, Barre, 360) classes.

AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM Rockland to initiate a charge to the credit card number entered into Pike13 or current point of sale system for my dues. This authorization is to remain effective until I have canceled my membership through the FIT4MOM franchise owner.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a return fee of $25. Members are responsible for updating their card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be canceled.

CANCELLATION POLICY
To cancel this membership within the first week, the member must contact the FIT4MOM franchise owner (rockland@fit4mom.com) in writing. This will terminate the contract and no month-to-month contract will be initiated. To cancel the monthly membership after the first week, the member must contact the FIT4MOM franchise owner (rockland@fit4mom.com) no less than 15 days prior to their next payment due date to request a cancellation. No refunds will be given for dues already paid. This membership is not transferable to another person or anywhere outside the FIT4MOM Rockland franchise.

HOLD POLICY
To place a hold on this membership, the member must contact the FIT4MOM franchise owner (rockland@fit4mom.com) in writing. Memberships may be held a maximum of 4 months per hold period. Hold starts will coincide with the next billing date and can end on any day of the month. When a hold period is ended, the monthly auto payments will resume on the return date. There will be a $10 processing fee charged to your credit card on file for the hold period. The hold fee will be waived for prenatal clients or with medical note.

HOLIDAY CLOSURE
FIT4MOM Rockland will take an annual holiday break from 12/24-1/1. An On Demand challenge will be provided during this time.

FIT4MOM ON DEMAND MONTHLY SUBSCRIPTION
With your paid monthly membership you are given free access to FIT4MOM OnDemand. You will have unlimited access to OnDemand workouts. When your FIT4MOM membership is properly cancelled according to the Terms and Conditions, your OnDemand subscription will be automatically cancelled. If you would like to keep your OnDemand subscription, you can re-enroll using the same login information and purchase your desired paid subscription.
